Team Portrait Order Form Instructions

• Please print legibly and fill out all fields. This information is used for several purposes including --- printing on pictures and specialty items; mailing prints if you are unable to pick them up
• Please provide your child's name as well as your name.
• Additional prints and specialty items are available only if you order one of the packages. Team photos may be purchased individually without a package and are located in the additional prints section of the order form.
• Please bring this form, completed, with you to your child's team scheduled session.
• Please bring payment, cash or a personal check payable to Mike Dawson, with you to your child's team scheduled session to avoid delays in processing.
• A $10 surcharge will be added to all orders placed after the last day of pictures (October 10, 2009).
• Please allow 2-3 weeks for delivery. Your child's pictures will be given to your child's coach and passed out at games or practices.
